Student / Teacher Evaluation question print request form can be obtained at the Dispatch window in the IT building
on Wilson
Road.

NAME: Please PRINT your Name, date, Department, phone, WSU e-mail address, IT Procedure number, IRI and WSU mail code.
FORM: Fill in the number of forms you want to have printed.
- Identify which form you will be using, Pink (26 question), or Blue (50 question).
- Please attach the list of questions you would like to have printed on your forms, or e-mail the list as TEXT (ascii) to Operator@wsu.edu.
- Choose a Response Scale, or supply your own.
- Indicate the session and year.
