Caution - Optical Scanning

WARNING!

Any time that you put an over print on any of the scan forms, please be very careful that none of the printing ends up in the registration marks ( the black dashes on two edges of the document.) The scanner will be unable to resolve the form if there is anything printed into those two areas of the forms. This relates to all scan form types used by the NCS scanner.

We can now pre-print your questions directly onto the Student Teacher Evaluations (pink and blue) survey forms, saving you both time and expense.
